A delivery note is handed over at the door and comes back signed. Its job is evidence: these goods, at this address, on this date, received by this person, in this condition. That makes it different from a packing slip, which travels inside the box and is never signed. The delivered-lines table varies with the drop, so this is a Google Doc.

Leave room for what goes wrong

Deliveries are short, damaged and refused often enough that the note has to accommodate it. Give the condition and shortage notes real space rather than a single line, and put the signature block where a driver can get to it without turning the page. Everything above it comes from the dispatch record — note number, addresses, order and PO references, lines delivered — so a route can be generated as a batch overnight and the paperwork always matches what was actually loaded.

Common questions, answered

How is a delivery note different from a packing slip? +
A packing slip sits inside the carton and tells whoever unpacks it what should be there. A delivery note is handed over at the door and signed: it is evidence that goods were received, by whom and in what condition. That signature block, and the space to note damage or shortages, is the whole point of the document.
Can it carry the customer's purchase order number? +
Yes, and it usually must. Goods-in departments match a delivery against the PO they raised, so the note places both your order number and their PO reference from the delivery record. Without it, the delivery sits unmatched and the eventual invoice gets queried.
Can I generate a whole route at once? +
Yes. Filter the deliveries table to a date and a vehicle or route and one run produces a note per drop, in the order they will be made. Scheduling that run overnight means the driver's paperwork is ready before the loading starts.
Can I use my own delivery note design? +
Yes. Many customers' goods-in processes expect a specific layout, and duplicate-copy stationery often dictates where the signature block has to sit on the page. Rebuild that document in Google Docs as it stands, mark the addresses, references and the goods table as placeholders, and generate against your own version instead.
